The incoming mail server is  access.k12.wv.us

Your username is your COMPLETE  access.k12.wv.us e-mail address

The password is your password.

The smtp server is the server of your Cell phone provider.

The smtp server of cwmx.com   may work for an Iphone using AT&T.

You may want to set the configuration to NOT download the entire message unless you want to read it, and also to leave mail on the server.
There may be other options to consider so that e-mail does not occupy all your storage on the Iphone.

If you understand how IMAP works, you might want to use IMAP instead of POP  for incoming mail.  If you use IMAP, the incoming mail server is still access.k12.wv.us.
